For your convenience, please find the administrative forms to the right that will be needed by our practice. Please print the forms, fill them out, and bring them to your appointment. We also advise you to review your health benefits book, or call your health plan to understand your coverage. We accept many insurance plans. Please call your insurance company to verify Dr. Kalra as a provider for your plan. If you wish to visit Dr. Kalra outside of your insurance network we accept private payment. It is important to get any pertinent and relevant medical records to your visit to Dr. Kalra sent to him ahead of your appointment.

Appointments

Our practice will accept patients 16 years and older.  If you would like to make an appointment with Dr. Kalra, please call our office at (949)645-1967 and follow the appropriate prompts to reach a scheduler between 9:00am and 5:00 pm Monday-Friday. 

Please note, if you are an HMO patient, you will need to obtain an authorization from your primary care physician, prior to scheduling an appointment with Dr. Kalra.

Patients requesting an appointment for a colonoscopy and/or other procedure, will need to make an appointment with Dr. Kalra or his assistant for a consultation.  Following an initial consultation, an appointment for a procedure will be coordinated by our office staff.

If you need to cancel an existing appointment, please call our office 24 hours in advance so that we may accommodate our other patients who need to see Dr. Kalra.

Medication Refills

If you received your prescription from Dr. Kalra, please call your pharmacy to request a renewal of your prescription.  This is the most efficient way to get your prescription refilled.  Your pharmacy will contact us to renew your prescription.

Please keep in mind, you will need to request a medication refill at least 5 business days before your prescription runs out.  All patient charts are reviewed prior to authorizing refills, therefore, our office will need an adequate amount of time to receive approvals.  Please do not wait until your prescription runs out as it may cause a break in your medication therapy.
